It’s 30 October 1988, and Michael Myers has been in a coma since his pursuit of Laurie Strode was finally stopped ten years ago, in Halloween II (1981).
While being transferred from Richmond Mental Institute to Smithsgrove Sanitarium, Myers wakes and hears that he has a seven-year-old niece in Haddonfield (the daughter of Laurie Strode). He kills the transfer crew and escapes.
In Haddonfield, the niece, Jamie (Danielle Harris), has been adopted by the Carruthers family but keeps having nightmares about Michael (although she is unaware who he is).
On Halloween night, Jamie goes out trick or treating, little knowing that her murdering Uncle is following her and her step-sister, Rachel (Ellie Cornell).
Rushing to her aid is Dr Loomis (Donald Pleasence) who begins to search the town with Sheriff Meeker (Beau Starr) to find Michael and protect Jamie. After finding Rachel and Jamie, they all hole up back at the Meeker’s house, completely unaware that Michael been hiding out in the back of the police car and is inside the house with them.
From here on out, it’s a pretty thrilling game of cat and mouse as Michael ends up chasing Rachel and Jamie onto the roof of the house, then to the local school and finally, Michael is hiding under the truck that’s meant to take them out of town.
This all leads to a showdown between Michael and the state police who essentially blow him to kingdom come.
